Work injuries may require a referral (this

be determined at the consultation).
Dr. Haddad is a preferred provider for most
private insurance and normally does not
require a referral.
If you have Group Health you may require a
referral from your primary physician. It is typically an easy and fast process.
We will determine if you require a referral prior to any charges incurring.
During your initial consultation, our staff
will determine your insurance coverage, co-pays and deductibles.
