To refer a patient for evaluation and care to Spokane Spine Center, simply
call the office at (509) 487-8000. Or you can download, print, and fax the referral form available on this website to (509) 487-6333.

Following the initiation of therapy, chart notes and a four week re-evaluation will be forwarded to the referring physician. At the conclusion of therapy, the patient will be discharged with a home exercise program they will use to maintain their progress. Typical treatment times are between 6-8 weeks.

If requested, Dr. Haddad is available for brief presentations at the referring physician's office. Visits to Spokane Spine Center are welcome and will include a brief tour and demonstration of the rehabilitation program.
